Loss of 2 Warbirds in Texas

Its being reported that a midair collision between a B17 and a P 63 just occurred at a Texas air show.

Just saw online a small video in an article of a Belgian newspaper.

Looks like a a P-63 flew through a B-17 just aft of the wings, and the detached rear fuselage of the B-17 can be seen falling just before the rest of the B-17 exploded in a large fireball.

Horrific, a complete lack of situational awareness.

 

I was a Human Factors in Military Aviation Facilitator for my last 8 years in the RCAF and just viewing these videos reveals a lot of links in the chain of events that led to this accident. 
 

There is always a chain of events and not just one cause of an accident. Is the system that broken that this preventable accident could happen?
